A prescription from your physician or other eligible provider is required (prescriptions can be faxed to 314-567-0569. Please have your insurance information available when calling.
New Patients
If you are a new patient, please arrive 15 minutes before your appointment to complete registration forms and a health history. For your convenience, you may print and complete our ambulatory summary list (a record of problems and procedures), then fax the list to the clinic at 314-567-0569, or bring it to your appointment.
