Unique to many outpatient settings, we only provide a one-patient to one-therapist ratio to ensure that you receive the hands-on, personalized attention necessary to your recovery. This hands-on, patient-centered approach is built upon the foundation of individualized treatment plans. These plans identify goals that are most meaningful to you.

After Therapy

At the conclusion of a therapy or rehabilitation program, many patients find it difficult on their own to maintain the progress they’ve made. Often, this can lead to decreased motivation, decline and injury. We designed our Fitness and Independence Training (FIT) Program to help patients transition by continuing with the guidance and expertise of a certified personal trainer. Fees for FIT are out-of-pocket and a physician’s referral is not required.

How To Make An Appointment

For appointments or additional information, please contact the Center for Outpatient Therapy and Wellness at 314-996-3500. A prescription from your physician or other eligible provider is required (prescriptions can be faxed to 314-567-0569. Please have your insurance information available when calling.

New Patients

If you are a new patient, please arrive 15 minutes before your appointment to complete registration forms and a health history. For your convenience, you may print and complete our ambulatory summary list (a record of problems and procedures), then fax the list to the clinic at 314-567-0569, or bring it to your appointment.
